  • The collection of PHI is essential to provide clients with the appropriate services and support.
  • LCMH uses a secure electronic system to share PHI with other health service providers within our agency to ensure coordinated care and services.
  • By agreeing to share their PHI, clients allow LCMH to use the information for the following purposes:
  • Providing health support and services based on client needs.
  • Maintaining up-to-date and complete records of clients' health history and needs within their circle of care.
  • Identifying gaps or overlaps in services to provide targeted assistance where needed.
  • Ensuring clients receive the right support and services.
  • Supporting other organizational activities to promote safe and quality services.


3. Disclosure of PHI to Third Parties:


  • LCMH may share PHI with other health service providers within our agency who are involved in providing support to the client now and in the future.
  • A client's instruction to withhold certain information cannot prevent LCMH from recording information required by law, professional standards, or best practice.

  • The PHI collected in clients' records belongs to them.
  • LCMH is committed to safeguarding the privacy and protection of clients' PHI.

  • All health service providers at LCMH have signed confidentiality contracts to ensure the privacy of client information.


3. Logging and Monitoring:


  • A log will be maintained to record instances when a person views clients' PHI. Regular review of this log will prevent unauthorized access to client information.


4. Retention and Disposal:


  • PHI will be stored and disposed of in compliance with relevant laws and regulations.


5. Investigating Breaches:


  • LCMH will promptly investigate any suspected breach or unauthorized access to clients' PHI and take appropriate action.
