Case Management

LCMH Case Managers provide case management services to individuals with severe and persistent mental illness within a community setting. Case management is a collaborative and client-directed process that allows an individual to work with a case manager to develop a care plan and work towards goals related to their recovery. Goals are determined by the individual and may relate to: mental health, physical health, financial, legal, relationships, employment, education and housing. The case manager works with the individual to connect them to resources in their communities that will support them in accomplishing their goals.

Emergency Room Diversion Case Management

LCMH Case Managers works as a member of a multi-disciplinary team and is responsible for providing short-term (up to 6 weeks) case management services to transitional-aged youth and adults who are experiencing mental health and/or substance use concerns and have presented to the Emergency Department. This position works in close partnership with Emergency Departments and Crisis Intervention services to provide assessment, coordinated care plan development, intervention, supportive counselling, service coordination and brief follow-up. The Case Managers provide timely emergency room diversion case management services, with no waiting list.

Social Recreation

The LCMH Recreation Facilitator provides quality programs and activities that support the client’s needs as well as provide opportunity for social support through Social Recreation Groups. One group is offered in Smiths Falls and another in Carleton Place. The program assists clients in reducing social isolation, assists in linking them to supports in the surrounding communities, and enhances their quality of life through social and physical participation. The Social Recreation Facilitator works in partnership with The Link (The Mental Health Support Project), a consumer support driven activity center. This integration of services supports both agency members/clients. There is a men’s support group, coordinated special events and activities for all members or clients.

Court Diversion & Support

LCMH offers both court support and court diversion services for those who have severe and persistent mental illness and are in conflict with the law. Court support provides case management services to individuals who may be eligible for a court related intervention. Court diversion also provides case management services with the intention of diverting individuals who have been charged with a minor criminal offence away from the judicial system and towards health care and social services.
